Management of Long Term Conditions

The Practice Nurses undertake the routine management of patients with Diabetes, Asthma, COPD, Heart Disease, Stroke/TIA and High Blood Pressure.

If you have a Long Term Condition (LTC) you will remember we liked to review you during the month of your birth.  As restrictions are slowly lifting we are about to resume seeing all of our patients who have a Long Term Condition.  During the pandemic we saw many of you, particularly those who we felt were at a higher risk, however we will now start to see all of you again.  Please call to book your appointment with our nursing team.  Asthma patients will be reviewed in a slightly different format so our reception girls will be able to guide you through the process when you call.

Please do not wait for us to invite you, call the team on 01872 863221.  We look forward to seeing you all again soon.  (Remember to bring a specimen of urine with you when you attend your LTC appointment)

BLOOD PRESSURE READINGS

We ask patients who are monitored for High Blood Pressure only to come and see us once a year for their annual blood tests and health check.  Then at six months we like the patient to take some home blood pressure readings and drop them into the surgery.  Monitors are not expensive and can be bought online or at a local chemist.  Below is a short info film about the benefits of home monitoring. We also now have a blood pressure machine in our foyer which you can use and hand your readings to the receptionist.
