New Patient Registration

Please note all new registrations will require you to provide us with your NHS number which can be obtained from your previous practice.  (Failure to do so may delay your registration)

If you live within the area covered by the Practice ask for a registration form from Reception or use the Online Pre-registration form below.

As soon as your completed forms are returned your details will be put on to our computer system and an application will be made for your existing medical records to be transferred.

We will contact you to book an appointment for you to meet or chat on the telephone with one of the doctors or nurses. This is important in order to review your medical history and assess your current needs. If you are a carer or look after a husband, wife, relative, friend or neighbour or a veteran please let us know.

Please check that your address is within our area on the 'Practice Boundary'' page/map before you register with us, if your address is outside this area we will be unable to accept your registration. 

 


Online Pre-registration with the Practice

If you wish to pre-register please follow these instructions:

We may not be able to process your registration fully if we do not receive your NHS number and completed Health Questionnaire.

IMPORTANT: We cannot proceed until both Registration and Health Questionnaire forms have been completed and we will then continue with the full registration and request your records from your previous surgery. You must complete ALL 4 STEPS below.

1) Complete the online form Pre-registration Form then click 'Send'

2) The following forms must be completed to enable us to register you here at the surgery

Download (Enable editing and save with your information) and complete the Health Questionnaire (Adults 16yr and over) (Children 0 up to 16 yrs)

3) Return your completed Health Questionnaires to registrations.carnondowns@nhs.net. (This is because it can take a considerable time for us to receive your medical records)

4) To complete the registration process please bring photo i.d. (passport or driving licence) to the surgery and sign the pre-registration form.  During the pandemic you may also scan and email the i.d. to the address above with the health questionnaire.  If you do not have i.d. then please ask to speak with one of the managers and we can help you get registered here at the surgery.
