Management of Long Term Conditions

The Practice Nurses undertake the routine management of patients with Diabetes, Asthma, COPD, Heart Disease, Stroke/TIA and High Blood Pressure.

If you have a Long Term Condition (LTC) you will remember we liked to review you during the month of your birth.  As restrictions are slowly lifting we are about to resume seeing all of our patients who have a Long Term Condition.  During the pandemic we saw many of you, particularly those who we felt were at a higher risk, however we will now start to see all of you again.  Please call to book your appointment with our nursing team.  Asthma patients will be reviewed in a slightly different format so our reception girls will be able to guide you through the process when you call.

Please do not wait for us to invite you, call the team on 01872 863221.  We look forward to seeing you all again soon.  (Remember to bring a specimen of urine with you when you attend your LTC appointment)

First Contact Physio

‘Musculoskeletal conditions’ is a broad term, encompassing around 200 different conditions affecting the muscles, joints and bones.

These conditions are a big problem in the UK; with over 17 million people living with a musculoskeletal condition, more people suffer with disability from musculoskeletal conditions than anything else.  Conditions of the bones, joints and muscles represent a significant cost to the individual, the economy and health and social care.

Good health of bones, joints and muscles underpins living life well. Musculoskeletal conditions affect nearly everyone at some point in their life. They can cause pain, fatigue, restricted mobility and activities of daily living. They impact people’s lives, their work and even other health conditions.

Bryony Short, Clare Platt and Rowena Beckley are our Musculoskeletal (MSK) Practitioners and are based on the North Coast at either Perranporth or St Agnes Surgery.  They all have extensive expertise in the clinical assessment, diagnosis and management of MSK (muscle or joint) conditions.

They see patients, aged 16 and over, with (suspected or diagnosed) MSK conditions, as the first point of contact, without need for a GP referral.

Outlook Southwest-Counselling Service

Outlook Southwest see patients who may be having difficulties with stress, depression, anxiety and/or other problems and who have been referred to them by the GP.

They provide different approaches for different types of problems. Some people are offered information, advice and practical suggestions for ways of helping themselves while others are offered the choice of seeing a psychological therapist for a number of appointments.

The available options and your own choices will be discussed with you in the first appointment. Generally first appointments last from between 30 to 45 minutes.

The therapists give your GP a brief summary of their work with you – you are welcome to see this. If there is anything you would prefer your therapist not to tell your GP, let them know. However, therapists have a duty of care. This means that if they have serious concerns about any risk to yourself or others they will need to tell your GP.

Cancelling Appointments and wasted appointments lead to longer waiting times. Patients who miss or cancel at short notice more than one appointment will generally be discharged by Outlook Southwest.
